In this case, there is not currently evidence of severe protein –calorie malnutrition in that Alice is not medically unstable (normal BP, HR and temperature). Therefore it would be reasonable to commence treatment on an outpatient basis. The principle of management should be monitoring of weight and physical parameters, ongoing assessment of her psychological state, including mood and anxiety symptoms and to support the family.

There is good evidence that adolescents who are treated by a psychologist using Family Based therapy according to the Maudsley Model have a better outcome than with other psychological interventions. The GP could provide the medical follow-up to support the psychologist if a specialised eating disorder service is not available. If depression or anxiety are significant then these should also be treated, by psychological therapies or medication, often a selective serotonin reuptake inhibitor (SSRI).
